To meet the Centers for Medicare and Medicaid Services (CMS) COVID-19 health care vaccination requirement, UI Health Care faculty, staff, volunteers, students, vendors, and service providers are being asked to document their vaccination or an exemption.

The requirement also covers select university students and employees who interact with patients of UI Hospitals & Clinics, indirectly support clinical and patient care activities, or are enrolled in a course or program taught in a health care facility.

If you have been identified as a student or employee who meets the criteria, you will receive an email directing you to indicate your compliance through one of the following options:

  1. Submit your COVID-19 vaccination information through Employee Self-Service;
  2. Apply for a medical exemption; or
  3. Submit an attestation for a religious or strongly held belief exemption.

Note: If you received your full COVID-19 vaccination series through the University Employee Health Clinic, your information will be automatically uploaded to the system and no further action is necessary.
